2 May 1916 Leading Seaman John McWalter MMR discharged dead – accident. He is buried in Sheerness (Isle of Sheppy) Cemetery, Kent – Grave PP586

4 April 1941 engaged in salvaging an RAF Blenheim aircraft from 27 Squadron when, through an error in navigation, she hit a mine and was sunk in the Singapore Roads with 26 killed and 19 injured. Eight of the deceased were British naval officers including the Chief Engineer Officer Colin Robert Parnell Barrett,  a Commander and a Lieutenant Commander Royal Navy. The others were two RAF Officers, a Warrant Officer and 22 other ranks, Asiatic seaman and dockyard employees. The minefield – laid by the Royal Navy – was known as No 3 Minefield and its position had been correctly recorded. The exact position of the crashed aircraft was uncertain. A board of enquiry found Lieutenant Commander Michael Beach Thomas, Royal Navy (deceased) responsible. Vessel broken in two.
